Welcome to Mayfair Medical Group

Mayfair Medical Group was founded in 1971 by Drs. Frank Waldo, Harry Register and Harry Bagby. Dr. Tommy Amason joined the practice soon after in 1972, and Mayfair was purchased by Children’s of Alabama in 1997.

Our eight pediatricians provide care for children from birth through college years. Our goal is to make your visits to our office as pleasant and stress-free as possible while we share in your child's care. Our staff is well trained to provide for your child’s medical needs, including well and sick visits, immunizations and school forms. If your child needs more specific treatment beyond a general pediatrician, we can refer you to highly qualified specialists in the Birmingham area.

Mayfair Medical Group was proud to receive Patient Centered Medical Home certification from the National Committee for Quality Assurance (NCQA) in 2017. The NCQA Patient Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduce costs. Mayfair was recertified in 2020.

Practice Information

All office visits are by appointment. Our goal is see your sick child on the day that you call our office, however there could be times we cannot meet this goal due to the high volume of sick children in the fall/winter months. Well check up appointments can be made in advance. The Saturday hours are for sick child visits only and are not scheduled in advance. If your child is needing immunizations only and is current on their well visits you may call the office and schedule a nurse only visit. The nurse only visits are scheduled on Tuesdays, Wednesdays, and Thursdays between 9:00am-12:00pm and 2:00pm-4:00pm. We schedule nurse only visits to help avoid long waiting times but please remember that our nurses are also assisting the physicians and patients in the office.

Prior to your first visit send us a copy of your child's immunization record and medical history. Please bring your insurance card to every visit.

Check-up Schedule:

  • 1 Month--- Check-up; Repeat PKU (State Newborn Screen)
  • 1 Month--- Check-up; Hepatitis B Vaccine
  • 2 Months--- Check-up; Pentacel/Prevnar/Rotavirus Vaccines
  • 4 Months--- Check-up; Pentacel/Prevnar/Rotavirus Vaccines
  • 6 Months--- Check-up; Pentacel/Prevnar/Rotavirus
  • 9 Months--- Check-up; Hepatitis B Vaccine/Vision/Lead Screens; Hematocrit (blood test for anemia)
  • 1 Year--- Check-up; Prevnar/Varicella/MMR Vaccines
  • 15 Months--- Check-up; Hepatitis A Vaccines
  • 18 Months--- Check-up; Pentacel Vaccine
  • 2 Years--- Check-up; Hepatitis A Vaccine
  • 3 Years--- Check-up; Vision Screen
    4 Years--- Check-up; DTaP/Polio/MMR/Varicella Vaccines
  • 5 Years--- Check-up; Vaccines not recieved at 4 years of age
  • 6 Years and older---Annual Check-up to include recommended immunizations or boosters as needed.

This summarizes the basic procedures that each well child visit includes. The height and weight will also be measured and you will receive a note card that has your child's measurements on it.

At all check-ups, you will receive hand-outs regarding Well Child Care and Normal Development. You will discuss with your child's doctor how your child's growth compares with other children his/her age.

Your child's doctor will remind you when your child's next check-up should be at each well child visit. Please don't hesitate to contact us at Mayfair if you have any questions.

Immunizations:

  • Hepatitis B - protects against serious liver diseases.
  • Rotavirus - protects against the most common cause of diarrhea and vomiting in infants and young children.
  • DTaP - protects against diphtheria, tetanus (lockjaw), and pertussis (whooping cough).
  • Tdap - protects teens and adults from tetanus, diphtheria, and pertussis.
  • Hib - protects against Haemophilus influenzae type b (a major cause of spinal meningitis).
  • Varicella - protects against chickenpox and its many complications including flesh-eating strep, staph toxic shock, and encephalitis (an inflammation of the brain).
  • MMR - protects against measles, mumps, and rubella (German measles).
  • Hepatitis A - protects against serious liver diseases.
  • Pneumococcal - protects against bacterial meningitis and infections of the blood.
  • Influenza - protects against flu.
  • Meningococcal - protects against very serious bacterial diseases that affect the blood, brain, and spinal cord.
  • HPV (human papillomavirus) - For girls and women to protect against the viruses that cause genital warts or that may cause cervical cancer.

Well Child Visits and Immunizations

Well child visits and immunizations are a critical part of keeping your child healthy. At these visits your pediatrician will not only be looking for signs of illness or disease, but we also want to ensure that your child's development is appropriate for his or her age. We welcome your questions about your child's illnesses, diet, development, behavior and any other concerns.

Usually our well child visits are scheduled for age 1 week, 1 month, 2 months, 4 months, 6 months, 9 months, 12 months, 15 months, 18 months, and 2 years. We continue to see children on a yearly basis until age 6 and every year thereafter until age 18. More frequent routine examinations might be necessary for children with chronic diseases or on daily medications.

Immunizations are typically coordinated with well child visits and are a very important part of preventative care. These vaccines help prevent a number of potentially serious and life-threatening diseases. The American Academy of Pediatrics recommends vaccines on a particular schedule. The most current recommendations can be found by clicking the “FAQs and Resources” tab in the top right corner under the search bar.

Well child visits are scheduled by appointment only during our regular office hours: 8:30 a.m.–5:00 p.m. Monday- Friday. We allow more time for our well child visits so that there will be plenty of time for you to discuss any questions you have about your child's health and development. However, if you have special concerns you wish to discuss at check-up time please alert our staff when you schedule your appointment so we can schedule adequate time for your visit.


Sick Visits

We see patients by scheduled appointments only. Receptionists begin answering phone calls at 8:00 a.m. Monday through Friday and will schedule appointments between 8:30 a.m. and 5:00 p.m. We are also open on Saturday mornings for acute medical problems from 9:00 a.m.–12:00 p.m. We start answering the phone at 8:30 a.m. on Saturdays.

Please note, you may be charged a $25 no-show fee for any appointments that you miss without calling to cancel within 4 hours of your scheduled appointment time.


Prescription Refills.

Prescription refills can be requested by calling our office during regular office hours (8:00 a.m. to 5:00 p.m. Monday through Friday), 205-870-1273. All refill requests must be reviewed by your child’s physician to determine if the refill is medically appropriate and/or if your child is up-to-date on required physical or medical management exams. We do request 24 hours to process all prescription requests. Refills for certain classes of medications cannot  be sent directly to your pharmacy and must be printed and physically signed by your child’s physician. These prescriptions must be picked up at our office.”

Our primary interest at Mayfair Medical Group is in taking care of your child. However, we must enforce our payment/financial policy in order to continue to provide your child the best care possible.

Professional fees are due and payable at the time service is rendered. Responsibility for payment is your direct obligation-not your insurance company's. Your co-payment and payment for non-covered services is expected at the time of service.

It is the patient/caregiver's responsibility to understand their insurance benefit plan. It is the patient/caregiver's responsibility to know if a written referral or authorization is needed to see specialists, if preauthorization is required prior to a procedure and what services are covered.

If a patient/caregiver has no insurance, they will be seen on a cash basis.

When you arrive at Mayfair Medical Group, you will sign an agreement to pay for any services rendered that are not covered by your insurance. Because we deal with dozens of different insurances, we do not have the ability to track each individual plan. It is very important to check with your insurance before making a well child appointment. Not all plans cover well child check-ups, annual/sports physicals, immunizations, routine labs, or hearing and vision screenings. If any benefit is not covered, it will be the responsibility of the patient/caregiver to make payment at the time of service.

Not all medically necessary services ordered by our physicians are covered by every insurance plan. If we need to send out lab tests that we do not routinely do in our own lab, we use LabCorp as our referred testing lab. Most major insurances will pay for labs/testing done by LabCorp. Please check with your insurance company to make sure that your insurance accepts LabCorp. If your insurance does not accept LabCorp, please let us know ahead of time as you may be charged by LabCorp for non-covered labs. If radiology services are necessary please be aware that radiology centers may bill for services that are applied to your deductible, or are otherwise uncovered. It is important that all patients/caregivers be familiar with the details of insurance plan coverage. Any service ordered by our physicians and not covered by insurance will be the responsibility of the patient/caregiver.

There is a $25 fee for all returned checks.

A $25 charge will be assessed to your account if appointments are not kept or rescheduled 24 hours in advance of appointment time.

After three unkept/uncancelled appointments, as described above, the physician reserves the right to discharge the patient from the practice.

A $5 processing fee will be charged for all immunization (blue card), physical, sports, and camp form when completed outside of an office visit with the physician. This action is necessary to help offset administrative costs associated with the processing of the form.

You should notify our billing office if you need to make special arrangements. An account overdue for 90 days may be referred to an outside collection agency for collection, so please keep us informed of difficulties when they arise. Once your account has been turned over for collection, we will no longer be able to serve your child's medical needs.

Divorced Parents

In cases of divorce and /or separation, the person bringing the child in for services will be obligated to pay for any co-pays or balances that are due. The legal guardian and /or the person bringing the child in for services will be held responsible for paying any balance resulting from that visit. If legal documentation is presented that someone other than the legal guardian is financially responsible, and accurate billing information can be provided, we will attempt to bill that party. Ultimately, in the State of Alabama, both biological parents are responsible for debts incurred by their minor children.

Registering Your Child

To access your child's patient portal, you must be registered by Mayfair Medical Group. To register, you must either request a registration in the office or call our office. Once you have been registered by Mayfair Medical Group, you will receive an email confirmation with a link to set up your account. You will enter YOUR information (not the patient’s) to set up the account. Please allow 24 hours for your account to be activated once your child has been registered.

At each visit you will be asked to sign several forms.

ADHD Forms

These are scoring sheets used to assess a child for ADHD.  These are screening tools, and as such, are only a part of the puzzle in figuring out which child might be at risk for ADHD.  The process is completed by your doctor at an office visit.  At the visit, your child's school and home history, medical history, and an exam are combined with the answers on these forms to assist in arriving at a diagnosis.

If this is the first time we are assessing your child for ADHD, you would want to print and complete the Initial Screening Forms below.  One or both parents might want to complete a form, and two or more primary teachers might be asked to complete a form as well.  The completed forms should be sent to your child's doctor in advance of the visit.


If your child's doctor needs to see how the medication is working, a follow-up form might be requested.  Parents and primary teachers should complete these forms.  The completed forms should be sent back to your child's doctor for review.  The parent and child might be asked to come to the office for a re-assessment after the forms are reviewed.
